Protected Health Information: Our office keeps Protected Health information records (PHI) for all our clients. We maintain your PHI in a secure location that cannot be accessed by anyone other than authorized persons within our office. If you would like a full copy of this offices Notice of Privacy Practices, one will be provided to you upon request.
Exceptions to confidentiality include:
- Suspected child abuse or dependant adult or elder abuse, for which I am required by law to report this to the appropriate authorities immediately.
- If a client is threatening serious bodily harm to another person/s, I must notify the police and inform the intended victim.
- If a client intends to harm himself or herself, I will make every effort to enlist their cooperation in ensuring their safety. If they do not cooperate, I will take further measures without their permission that are provided to me by law in order to ensure their safety
Due to new FTC rules regarding Identity theft, our new clients are required to provide our office with photo identification. Our office has policies in place to identify, detect and respond to possible identity theft.
